Forms to Self-Prove an Alabama Will

Use this form to self-prove your will at the same time as signing your last will and testament. Your and each witness’s signature must be witnessed by an “officer authorized to administer oaths under the laws of the state where the execution occurs”. The officer’s witnessing must be evidenced by the officer’s certificate (signature) under official seal (43-8-132(a):

“I, _____, the testator, sign my name to this instrument this ______ day of _____, 19__, and being first duly sworn, do hereby declare to the undersigned authority that I sign and execute this instrument as my last will and that I sign it willingly (or willingly direct another to sign for me), that I execute it as my free and voluntary act for the purposes therein expressed, and that I am 18 years of age or older, of sound mind, and under no constraint or undue influence.”

If you already have a last will and testament in Alabama, you may still have this will self-proved by using the following form. This form has the same execution requirements as the form above and must be “attached or annexed to the will” (43-8-132(b):
